Select a monster from the dropdown list, and click the corresponding button to load it into a slot. You can also type in its number manually. Entering "0" in a box will clear that slot.
Values in (parentheses) are stats after leader skills have been applied, but as of yet this does not factor in the extra damage done by the second elemental attack gained by an Ultimate Evolution.
